Transcription: 

Charley Baldwin sworn says he works in the factory with Mr Lucas, yesterday morning we were unloading Lumber from a car. Andrew Dawkins was pulling and sliped [sic] he fell backwards, I saw he was very badly hurt. We did what we could for him, put him in a shade place. No body but himslef to blame it was purely accidental.
